Error code
I7 — Service date does not match patient enrolment date
Date doesn't match enrolment
The service date doesn't match the patient's enrolment date; codes like Q200A and Q023A must use the enrolment form's date.
What to do: Find the signed Patient Enrolment and Consent form and compare its date with the claim and the roster report.
- Code
- I7
- Description
- Service date does not match patient enrolment date
Explanation
Updated October 26, 2026
I7 means the service date on the claim doesn't match the patient's enrolment date. Check the date on the enrolment form and ask for a review if needed.
What it means
I7 means the date of the claim does not match the date the Ministry has as the patient's enrolment date. Some primary care codes must carry the enrolment date as their service date.
For example, the rostering code Q200A and the unattached-patient fee Q023A use the date on the Patient Enrolment and Consent form as the service date.
Common causes
- The service date on an enrolment-related code is not the date on the signed enrolment form
- The enrolment was processed with a different date from the one billed
- A typing error in the service date
How to fix it
- Find the signed Patient Enrolment and Consent form and note its date.
- Compare it with the service date on the claim and with the enrolment date in the Ministry's roster report for the patient.
- If the claim date was wrong, ask the Ministry how to correct it through a Remittance Advice Inquiry (RAI).
- If you believe the decision is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.
Don't resubmit if
- The line is already on your RA. Question it with an RAI, not a new claim.
- You can't find a signed enrolment form for the date you billed.
How APEX handles it
