Error code

EP3 — Incorrect service date - check enrolment date

Wrong enrolment date

The service date on the enrolment claim doesn't match the patient's enrolment date.

What to do: Set the service date to the date on the signed enrolment and consent form, and resubmit.

Code
EP3
Description
Incorrect service date - check enrolment date

Explanation

Updated October 26, 2026

EP3 means the service date doesn't fit the patient's enrolment date. Use the date on the signed enrolment form and resubmit.

What it means

EP3 means the service date on the claim is wrong for the patient's enrolment, and the Ministry asks you to check the date of enrolment.

For an enrolment code such as Q200A, the service date must be the date on the Patient Enrolment and Consent to Release PHI form. Since February 1, 2013 that form stays in the office (OMA Q-codes guide p.9 to 10).

Common causes

  • The enrolment claim used the visit date instead of the date on the enrolment form
  • A typing error in the service date

How to fix it

  1. Find the patient's signed enrolment and consent form.
  2. Set the service date on the claim to the date on the form.
  3. Resubmit.

Don't resubmit if

  • The service date already matches the form. Look into the patient's enrolment status instead.
