Error code
F6 — Allowed as Minor/Partial Assessment
Paid as minor assessment
OHIP paid the assessment you billed at the lower minor or partial assessment fee.
What to do: If your record supports the higher assessment, send a remittance advice inquiry with chart notes within 7 months.
- Code
- F6
- Description
- Allowed as Minor/Partial Assessment
Explanation
Updated October 26, 2026
F6: the assessment was paid at the minor or partial assessment fee instead of the code you billed. Check the record supports the higher level.
What it means
F6 means OHIP reduced the assessment you billed and paid it as a minor or partial assessment, a lower fee.
Common causes
- The assessment billed needs elements that the claim or record did not support
- A Schedule rule limits that assessment for this patient or setting (for example, a limit already reached)
How to fix it
- Check the requirements for the assessment you billed in the Schedule of Benefits.
- If your record supports the higher assessment, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with the chart notes.
- If not, the reduced payment stands.
Don't resubmit if
- The claim is already on your RA. Use an RAI, not a new claim, if you disagree.
