H9 — Concurrent care already claimed by another doctor
Concurrent care already claimed
Another physician already claimed concurrent care for this patient, so your concurrent care claim wasn't paid.

- Code
- H9
- Description
- Concurrent care already claimed by another doctor
Explanation
Updated October 26, 2026
H9: another physician has already claimed concurrent care for this patient, so this concurrent care claim wasn't paid.
What it means
H9 means OHIP did not pay your concurrent care claim because another physician had already claimed concurrent care for the same patient.
Concurrent care is a routine in-hospital assessment by a consultant after their first major assessment, when the family physician stays the most responsible physician and asks the consultant to keep directing care. Supportive care is a routine in-hospital visit by a family physician who is not treating the case, at the patient's or family's request, for liaison or reassurance. Each is limited to 4 claims in the first week and 2 per week after that (Schedule GP48).
Common causes
- Two consultants both billed concurrent care for the same patient and period
- Concurrent care billed after another physician already took that role
How to fix it
- Check which physicians were giving directive care to the patient.
- If both roles were real and separate, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, explaining each physician's role.
Don't resubmit if
- The claim is already on your RA. Use an RAI, not a new claim, if you disagree.
