Error code
I8 — Confirmation not received
Confirmation not received
OHIP didn't pay this line because a confirmation it needed wasn't on file; the Ministry doesn't say which one.
What to do: Check the program or premium the fee code belongs to and whether its registration or enrolment was in place.

Explanation
Updated October 26, 2026
I8 means OHIP did not receive a confirmation it needed to pay this line. Find out what confirmation applies and ask for a review.
What it means
I8 is a Ministry message that reads only "Confirmation not received". OHIP did not pay the line because a confirmation it needed was not on file.
The Ministry's code list does not say which confirmation is meant. It is usually tied to the program or premium the line belongs to.
How to fix it
- Look at which fee code carries I8 and what program or premium it belongs to.
- Check whether that program needs a registration, enrolment or other confirmation from you or the patient, and whether it was in place on the service date.
- If you can't tell what is missing, contact the Ministry's claims office for your area.
- If you believe the decision is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.
Don't resubmit if
- The line is already on your RA. Question it with an RAI, not a new claim.
