Code
L4
Description
Not allowed to attending physicians

Explanation

Updated October 26, 2026

L4 means this lab service isn't payable to the attending physician. It is billed by the laboratory instead.

What it means

L4 means the laboratory service billed is not payable to an attending physician (the physician caring for the patient). It is a laboratory benefit.

Common causes

  • A physician billed a lab code that is payable only to a licensed laboratory

How to fix it

  1. Check whether the code is a laboratory code or an office lab code that physicians may bill (for example G001 or G002 in the physician Schedule).
  2. If it is a laboratory-only service, it is not payable to you.
  3. If you believe the decision is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.

Don't resubmit if

  • The code is payable only to a laboratory.
  • The line is already on your RA. Question it with an RAI, not a new claim.
