Code
M3
Description
Maximum prenatal care fee reached

Explanation

Updated October 26, 2026

M3 means the maximum fee for prenatal care has been reached for this pregnancy. Check prenatal visits already paid.

What it means

M3 means the maximum fee allowed for prenatal care has been reached. Further prenatal claims for this pregnancy are reduced or not paid.

The Schedule sets limits on prenatal services. For example, the antenatal preventative health assessment P005 is paid once per pregnancy, and the prenatal general assessment P003 is paid after another general assessment only if the first was not for starting antenatal care (Schedule K2 and K4).

Common causes

  • A prenatal service with a per-pregnancy limit billed more than once
  • Prenatal visits billed by more than one physician for the same pregnancy

How to fix it

  1. List the prenatal services already paid for the pregnancy, by you and other physicians.
  2. Check the limits for each code in the Obstetrics section of the Schedule (K2 to K4).
  3. If the visit was for an illness unrelated to the pregnancy, it may be billed as an ordinary assessment (Schedule K2, note 4).
  4. If you believe the decision is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.

Don't resubmit if

  • The prenatal limit was already reached.
  • The line is already on your RA. Question it with an RAI, not a new claim.
