What it means
O7 means the initial prenatal visit was already claimed, so this visit was paid at the subsequent prenatal visit fee.
A prenatal general assessment (P003) starts antenatal care. Later routine visits are minor prenatal assessments (P004) (Schedule K2 and K4).
Common causes
- A second P003 billed for the same pregnancy
How to fix it
- Check whether a P003 was already paid for this pregnancy.
- If the reduction is correct, there is nothing to fix.
- If you believe the decision is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.
Don't resubmit if
- P003 was already paid for this pregnancy.
- The line is already on your RA. Question it with an RAI, not a new claim.
