- Code
- O2
- Description
- Prenatal care already billed
Explanation
Updated October 26, 2026
O2 means prenatal care for this pregnancy was already claimed, so this service isn't paid as billed.
What it means
O2 means previous prenatal care has already been claimed, and the service billed can't be paid in addition. Some prenatal services are limited per pregnancy, such as P005, once per pregnancy (Schedule K4).
Common causes
- An initial prenatal service billed again in the same pregnancy
- P005 billed more than once
How to fix it
- Check the prenatal services already paid for this pregnancy.
- Later routine visits are billed as minor prenatal assessments (P004) (Schedule K2 and K4).
- If you believe the decision is wrong, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details.
Don't resubmit if
- The line is already on your RA. Question it with an RAI, not a new claim.
How APEX handles it
