MR — Minimum service requirements have not been met

Minimum services not met

The required minimum qualifying services weren't found, so this paid $0; usually Q040A without three K030A visits in 12 months.

What to do: Count K030A visits by the same physician in the prior 12 months and check at least one was in person.

Code
MR
Description
Minimum service requirements have not been met

Explanation

Updated October 26, 2026

OHIP paid $0 because the minimum services this code depends on weren't found. For family physicians it is usually Q040A without enough K030 visits.

What it means

MR is a Remittance Advice (RA) explanatory code meaning the minimum service requirements have not been met. The service is processed but paid $0.

Some fees are only payable after a set number of other services. The General Preamble says that when fewer than the required minimum services are rendered, the service is not eligible for payment (GP7).

In family practice the usual case is Q040, the diabetes management incentive ($65.70). It is only payable if you rendered at least three K030 diabetic management assessments for the same patient in the same 12-month period (A53). The Ministry also says Q040A is paid $0 with MR when none of those K030 visits in the preceding 12 months was in person.

Common causes

  • Q040A billed before three K030A visits were billed for the patient in the 12-month period
  • All of the K030A visits in the 12 months were virtual, with no in-person K030A
  • K030A visits billed under a different physician's number, so they don't count toward this physician's Q040A
  • Limited virtual care visits (A101A, A102A) counted toward a management fee; the Ministry says they don't count as qualifying services

How to fix it

  1. Count the qualifying services you billed for the patient in the 12 months before the incentive or management fee. For Q040A, that is K030A visits by the same physician.
  2. Check that at least one K030A in the 12 months was in person.
  3. If the minimum wasn't met, the fee isn't payable yet. Bill it again only once the requirements are complete for a 12-month period, as the Q040 claims submission instructions say (A53).
  4. If you believe the minimum was met, for example because a qualifying K030A was paid, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, and list the qualifying services.

Don't resubmit if

  • The required number of qualifying services still isn't on file for that 12-month period.
  • None of the K030A visits in the 12 months was in person.
  • You would be billing a K030A for a visit that didn't meet K030's own requirements just to reach the minimum.

How APEX handles it

APEX lists MR claims in the Correction Centre. For some family practice claims it drafts a corrected claim: check the drafted codes against the chart before you approve it. If you've turned on auto-fix, drafted corrections are sent without waiting for you. Other MR claims are listed under Outstanding for your input. See how APEX works · Pricing
