Error code
TM1 — Duplicate telemedicine claim for same patient
Duplicate telemedicine claim
OHIP already has a telemedicine claim on file for the same patient and service, so this one was rejected.
What to do: Find the original claim on your remittance advice; if it was paid, don't resubmit.
- Code
- TM1
- Description
- Duplicate telemedicine claim for same patient
Explanation
Updated October 26, 2026
TM1 means a telemedicine claim for the same patient and service was already submitted. Check for a duplicate before doing anything else.
What it means
TM1 ("Duplicate Telemedicine Claim, Same patient") means OHIP rejected the claim because it matches a telemedicine claim already on file for the same patient.
TM codes belong to the Ministry's telemedicine error codes (ET1 to ET5 and TM1 to TM8), which were tied to the former telemedicine program. For services on or after December 1, 2022, the virtual care funding framework replaced it: the 'OTN' service location indicator was discontinued (claims using it reject with V73) and OVCP/telemedicine B-prefix codes reject with A3E. Virtual care is now billed with the in-person code plus the K300A (video) or K301A (telephone) modality indicator, or A101A/A102A for limited virtual care.
Common causes
- The same telemedicine claim was sent twice
- A corrected claim was sent while the original was still on file
- Two physicians in the same group billed the same encounter
How to fix it
- Check your submissions and Remittance Advice for the original claim.
- If the original was paid, nothing more is needed.
- If this was a different service, correct the claim so it is clearly distinct (date, code) and resubmit. If the service date is on or after December 1, 2022, bill it under the virtual care rules instead: the Appendix J code plus K300A or K301A, or A101A/A102A, with no OTN service location indicator.
Don't resubmit if
- The original claim was paid. A second claim would duplicate it.
