- Code
- TM5
- Description
- Telemedicine claim already paid for same patient
Explanation
Updated October 26, 2026
A telemedicine claim was already paid for this patient for the same service. Check your RA before resubmitting.
What it means
TM5 ("Telemedicine Claim Paid for same patient") means a telemedicine claim for the same patient has already been paid, so this one conflicts with it.
TM codes belong to the Ministry's telemedicine error codes (ET1 to ET5 and TM1 to TM8), which were tied to the former telemedicine program. For services on or after December 1, 2022, the virtual care funding framework replaced it: the 'OTN' service location indicator was discontinued (claims using it reject with V73) and OVCP/telemedicine B-prefix codes reject with A3E. Virtual care is now billed with the in-person code plus the K300A (video) or K301A (telephone) modality indicator, or A101A/A102A for limited virtual care.
Common causes
- The same telemedicine encounter was billed twice
- Another physician in the group billed the encounter
- The service date is wrong on one of the claims
How to fix it
- Find the paid claim on your Remittance Advice.
- If it covers this encounter, nothing more is needed.
- If this was a separate service, correct the details and resubmit. If the paid claim is wrong, use an RAI.
Don't resubmit if
- The paid claim already covers this encounter.
