- Code
- V13
- Description
- Patient's date of birth is missing/invalid
Explanation
Updated October 26, 2026
The patient's date of birth is missing or not a valid date. Enter it as 8 digits and resubmit.
What it means
V13 ("Patient's date of birth is missing/invalid format") means the date of birth on the claim failed the Ministry's format check: it isn't 8 digits, the month isn't 01 to 12, or the day doesn't exist in that month.
This is a format check: the claim was rejected before OHIP assessed the service itself, so it usually pays once the field is corrected.
Common causes
- The date of birth was left blank
- Day and month entered the wrong way round
- An impossible date, such as 31 for a 30-day month
How to fix it
- Check the patient's date of birth against their health card or chart.
- Correct it in the patient's record so future claims are right.
- Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- You haven't changed anything. Sending the same claim again will reject the same way.
How APEX handles it
APEX reads your error reports and remittance advice from MCEDT and, for many claims rejected with this code, works out the fix for you to approve before it is resubmitted. See how APEX works · Pricing
