- Code
- V18
- Description
- Invalid Admission/First Visit date
Explanation
Updated October 26, 2026
The admission or first visit date on the claim isn't valid. Check the date and resubmit.
What it means
V18 ("Invalid Admission/First Visit date") means the admission date (or first visit date) on the claim is missing where needed, or not a valid date.
This is a format check: the claim was rejected before OHIP assessed the service itself, so it usually pays once the field is corrected.
Some services need an admission date on the claim, for example in-patient hospital visits and W010 monthly long-term care management (GP51–GP53).
Common causes
- The admission date was entered in the wrong format
- The admission date is after the service date
- A typing error in the date
How to fix it
- Check the admission date in the hospital or facility record.
- Correct it on the claim.
- Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- You haven't changed anything. Sending the same claim again will reject the same way.
How APEX handles it
