Code
V66
Description
Missing admission date

Explanation

Updated October 26, 2026

V66 means a hospital diagnostic claim used SLI code HIP but had no admission date. Add the patient's admission date and resubmit.

What it means

V66 means the claim was for a diagnostic service, the service location indicator (SLI) was HIP, and the admission date field was blank. The Ministry rejects it because a HIP claim needs the date the patient was admitted.

The service location indicator (SLI) is a three-letter code on the claim that tells OHIP where the service was provided. For hospital diagnostic service billings, the Ministry recognizes five hospital SLI codes: HDS, HED, HIP, HOP and HRP. Each of these also needs the hospital's master number on the claim.

Common causes

  • The admission date was not entered or not carried over from the hospital record
  • The SLI was set to HIP by mistake for a patient who was not admitted

How to fix it

  1. Get the patient's admission date for that hospital stay from the hospital record.
  2. Add the admission date to the claim. Check that the hospital master number is also on the claim.
  3. If the patient was not admitted on the service date, change the SLI to the code that matches where the service was done instead of adding a date.
  4. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.

Don't resubmit if

  • You resend the claim with SLI HIP and still no admission date. It will reject again.
