Explanation
Updated October 26, 2026
V67 means a hospital diagnostic claim used SLI code HIP but had neither a master number nor an admission date. Add both and resubmit.
What it means
V67 means the claim was for a diagnostic service, the service location indicator (SLI) was HIP, and both the hospital master number and the admission date were missing. It combines V65 (missing master number) and V66 (missing admission date).
The service location indicator (SLI) is a three-letter code on the claim that tells OHIP where the service was provided. For hospital diagnostic service billings, the Ministry recognizes five hospital SLI codes: HDS, HED, HIP, HOP and HRP. Each of these also needs the hospital's master number on the claim.
Common causes
- Hospital details were never entered for this patient or this facility in the billing software
- The SLI was set to HIP by mistake
How to fix it
- Confirm the hospital where the service was done and its master number.
- Get the patient's admission date from the hospital record.
- Add both to the claim. If the patient was not admitted, change the SLI to the correct code instead.
- Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- Only one of the two fields has been added. The claim will reject again as V65 or V66.
