Explanation
Updated October 26, 2026
V23 means OHIP questioned the number of services. It usually means a B- or C-suffix fee code was billed with only one service.
What it means
The Ministry's rejection list describes V23 only as 'Check number of services'. Its technical specifications set the matching rule: when a fee code ends in B or C, the number of services must be greater than 01, because those codes are billed in units. A short list of B and C codes are exceptions.
Fee code suffixes show who provided the service. The A suffix is the main physician's service, B is the surgical assistant's and C is the anaesthetist's. A family physician who did the service themselves normally bills the A suffix.
Common causes
- A B- or C-suffix code was billed with 01 in the number of services.
- The wrong suffix was keyed, for example B instead of A for your own service.
- Assistant or anaesthesia units were not added up before billing.
How to fix it
- Check the suffix on the rejected fee code.
- If you provided the service yourself rather than as an assistant or anaesthetist, change the suffix to A.
- If the B or C suffix is correct, enter the number of units calculated under the Schedule of Benefits. It must be more than 01 unless the code is on the Ministry's exceptions list.
- Resubmit the corrected claim. If more than three months have passed since the service date and the original claim was sent on time, use the stale-dated claim process described under VJ7.
Don't resubmit if
- Nothing has changed. A B- or C-suffix code with 01 services will reject again.
How APEX handles it
For family practice claims, APEX proposes resubmitting the visit as A007A (intermediate assessment) with the claim's first diagnostic code. Check that A007A matches the service before you approve it. V23 claims from other specialties are listed for you to correct. See how APEX works · Pricing
