V4 — Service date was not a weekend or holiday
Not a weekend or holiday
A weekend or holiday code, such as A888A or Q888A, was refused because the service date wasn't a Saturday, Sunday or holiday.
What to do: Check the date against the chart; if it was entered wrong, send a remittance advice inquiry, otherwise bill the weekday code.

- V4
- Description
- Service date was not a weekend or holiday
Explanation
Updated October 26, 2026
OHIP refused a weekend or holiday code because the service date wasn't a Saturday, Sunday or holiday. Check the date, then bill the weekday code.
What it means
V4 is a Remittance Advice (RA) explanatory code meaning the date of service was not a Saturday, Sunday or statutory holiday. It appears on codes that are only payable on those days.
In family practice the usual codes are A888A, the emergency department equivalent partial assessment ($44.55), which applies only on Saturdays, Sundays and holidays (A5), and Q888A, the FHO weekend and holiday access code for unscheduled visits (AQ2).
The Schedule defines holidays on page GP3. When a holiday falls on a weekend, the observed day moves to the Friday before or the Monday after. A888 and the H-prefix emergency department codes use a longer holiday list that counts December 25 to 31. For Q888A the Ministry announces the holiday dates each season; for 2024/2025 it was December 25 to January 1 (INFOBulletin 241201).
Common causes
- A888A or Q888A billed for a weekday visit
- The service date entered wrong, for example the Monday after a weekend clinic
- A holiday that isn't on the Schedule's list, or the wrong observed day when the holiday fell on a weekend
- Treating December 27 to 31 as holidays for codes other than A888A and the H-prefix emergency codes
How to fix it
- Check the service date against the patient's chart.
- If the date was entered wrong and the visit really was on a weekend or holiday, send an RAI asking for the date to be corrected.
- If the visit really was on a weekday, bill the weekday code that matches what you did. For a family practice office visit this is often an assessment such as A007A or A001A. Q012A can still apply to eligible after-hours weekday evening visits for enrolled patients.
- Bill within three months of the service date.
- If you believe the date was a recognized holiday, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date.
Don't resubmit if
- The service date is correct and wasn't a Saturday, Sunday or recognized holiday, and you'd be sending the same weekend code again.
- The visit's main service was already paid. Sending it again would be a duplicate.
How APEX handles it
APEX lists V4 claims in the Correction Centre. For some family practice claims it drafts a corrected claim: check the drafted codes against the chart before you approve it. If you've turned on auto-fix, drafted corrections are sent without waiting for you. Other V4 claims are listed under Outstanding for your input. See how APEX works · Pricing
