Error code
IA — Premium not payable with this fee code
Premium not allowed with code
A premium wasn't paid because the fee code it was attached to isn't on that premium's list of eligible codes.
What to do: Check whether the main service was paid, then compare the fee code with the premium's eligible codes.

- IA
- Description
- Premium not payable with this fee code
Explanation
Updated October 26, 2026
OHIP refused a premium because it can't be paid with the fee code it was billed with. Check the premium's list of eligible codes.
What it means
IA means a premium on the claim was not paid because the fee code it was attached to is not one that premium can be paid with. Premiums apply only to a set list of codes. For example, the primary care after-hours premium applies to listed assessment and counselling codes, not to every service.
IA is close to I6. I6 says a premium doesn't apply for another reason, such as the time or the patient; IA is about the pairing of premium and fee code.
IA often appears together with other codes. With V4, the date of service was not a Saturday, Sunday or statutory holiday. With B8, the service can't be paid when delivered virtually.
Common causes
- A premium billed with a fee code that isn't on its eligible list
- Weekend or holiday codes such as Q888A or A888A billed on a weekday (with V4)
- A premium on a virtual visit whose base service isn't payable virtually (with B8)
How to fix it
- Check which fee code the premium was billed with, and whether the main service was paid.
- Compare it with the premium's eligible fee codes in the Schedule of Benefits or your agreement.
- If the main service was paid, the premium simply doesn't apply and there is nothing to resubmit.
- If the whole visit came back at $0, a corrected claim without the ineligible premium, or with the right visit code, may be needed.
- If you believe the pairing is eligible, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date.
Don't resubmit if
- The main service was paid. Sending the visit again would duplicate it.
- The corrected claim would bill the same premium with the same fee code.
How APEX handles it
APEX fixes IA automatically in the Correction Centre: the corrected claim drops the premium (Q) codes and keeps the rest. With V4 it drops Q888A/A888A and uses A007A with Q012A if only premium codes are left. With B8 on a K300A/K301A virtual visit, it rebuilds the visit as A007A plus the modality code, adding Q012A on weekends and holidays. See how APEX works · Pricing
