Error code

V64 — Missing service location indicator

Service location missing

The service billed needs a service location indicator, and the claim had none.

What to do: Check where the service was done, add the matching service location indicator and resubmit.

Code
V64
Description
Missing service location indicator

Explanation

Updated October 26, 2026

The claim needs a service location indicator (SLI) and had none. Add the correct SLI and resubmit.

What it means

V64 ("Missing service location indicator") means the claim had no service location indicator, but the service billed needs one.

Common causes

  • The SLI was left blank
  • Billing software didn't send it

How to fix it

  1. Check where the service was done.
  2. Add the matching SLI.
  3. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.

Don't resubmit if

  • You haven't changed anything. Sending the same claim again will reject the same way.

How APEX handles it

APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
