- Code
- V62
- Description
- Invalid SLI for hospital diagnostic service
Explanation
Updated October 26, 2026
The service location indicator (SLI) for this hospital diagnostic service isn't one of the five valid codes. Correct it and resubmit.
What it means
V62 ("Invalid service location indicator") means a hospital diagnostic service billed by a participating hospital physician or group didn't carry a valid service location indicator (SLI).
The Ministry names five valid SLI codes for hospital diagnostic services: HDS, HED, HIP, HOP and HRP.
Common causes
- The wrong SLI was entered
- An office SLI was used for a hospital service
How to fix it
- Check where the service was done.
- Use the matching SLI (HDS, HED, HIP, HOP or HRP).
- Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- You haven't changed anything. Sending the same claim again will reject the same way.
