Error code
V69 — Service Date Invalid for SLI
Location code expired
The service location indicator on the claim wasn't valid for the service date, often because it was discontinued or the date is wrong.
What to do: Check the service date first, then use a location indicator that was in use on that date and matches where you worked.
- Code
- V69
- Description
- Service Date Invalid for SLI
Explanation
Updated October 26, 2026
V69 means the service location indicator on the claim isn't valid for that service date. Check the date and use an SLI that applies on that date.
What it means
V69 means OHIP did not accept the service location indicator (SLI) for the date of service on the claim. An SLI code can stop being valid from a given date. For example, the OTN SLI was discontinued for services on or after December 1, 2022 (that case has its own code, V73).
The Ministry's list gives no more detail than "Service Date Invalid for SLI".
Common causes
- An SLI that had been discontinued before the service date
- A wrong service date, which puts the claim outside the period the SLI was valid
- An SLI picked from an old template or default in the billing software
How to fix it
- Check the service date first. Fix it if it was typed wrong.
- Check that the SLI was in use on that date and matches where the service was done. Change or remove it as needed.
- Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- You have changed neither the SLI nor the date.
How APEX handles it
