Code
V98
Description
Wrong preventive care service date

Explanation

Updated October 26, 2026

V98 means the service date on a preventive care claim is wrong. Check the date rules for that preventive care code and resubmit.

What it means

V98 means OHIP rejected a preventive care claim because of its date of service. The Ministry's list gives only the short description "Wrong Preventive Care Date of Service".

Preventive care codes in the patient enrolment models include $0 tracking and exclusion codes (such as Q011A, Q130A to Q133A, Q140A to Q142A) and the cumulative preventive care bonus codes (Q100A to Q123A), which are measured against fiscal-year deadlines such as March 31.

Common causes

  • A service date that falls outside the period the preventive care code applies to
  • A typo in the date

How to fix it

  1. Check the service date against the chart and against the rules for that preventive care code.
  2. Correct the date.
  3. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.

Don't resubmit if

  • You don't know which date the code needs. Check before resending.

How APEX handles it
