VH6 — Mixed Service Dates

Mixed service dates

The service dates on the items of this claim are mixed in a way OHIP won't accept on one claim.

What to do: Check every item's service date and correct typos; if the dates are right, submit the items as separate claims.

Code
VH6
Description
Mixed Service Dates

Explanation

Updated October 26, 2026

VH6 means the service dates on the claim are mixed in a way OHIP won't accept. Check each date and split the claim if needed.

What it means

VH6 means OHIP rejected the claim for "Mixed Service Dates". The Ministry's list gives no further detail.

It is listed with the health number errors, next to VH5 (OHIP number required for service date) and VH7 (health number and OHIP number on the same claim).

Common causes

  • Items on one claim with service dates that need different patient identifiers or rules
  • A typo in one item's date

How to fix it

  1. Check the service date on every item of the claim.
  2. Correct any wrong date. If the dates are right, submit the items as separate claims.
  3. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.

Don't resubmit if

  • Nothing on the claim has changed.

How APEX handles it

APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
