Error code

VH7 — Health number and OHIP number on same claim

Two patient identifiers

The claim identified the patient with both a health number and an OHIP number, but OHIP accepts only one.

What to do: Keep the health number and version code and remove the OHIP number, unless the service date calls for one.

Code
VH7
Description
Health number and OHIP number on same claim

Explanation

Updated October 26, 2026

VH7 means the claim carried both a health number and an OHIP number. Keep only the identifier that applies and resubmit.

What it means

VH7 means the claim identified the patient with both a health number and an OHIP number. OHIP accepts only one patient identifier on a claim.

The Ministry's list gives only the short description "Health number and OHIP number on same claim".

Common causes

  • Both identifiers stored in the patient record and sent together
  • Old patient data imported into the billing software

How to fix it

  1. Check the patient's current card.
  2. Keep the health number and version code, and remove the other identifier, unless the service date calls for an OHIP number (see VH5).
  3. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.

Don't resubmit if

  • Both identifiers are still on the claim.

How APEX handles it

APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
