Error code

VHA — OHIP number not registered for this health number

OHIP number doesn't match

The OHIP number on the claim isn't registered against this patient's health number in the Ministry's records.

What to do: Correct the identifiers from the patient's current card; if no OHIP number is needed, bill with the health number and version code only.

Code
VHA
Description
OHIP number not registered for this health number

Explanation

Updated October 26, 2026

VHA means the OHIP number on the claim isn't registered with the Ministry for that health number. Check the patient's identifiers and resubmit.

What it means

VHA means the OHIP number sent on the claim is not registered with the Ministry against the patient's health number. The two identifiers don't belong together in the Ministry's records.

Common causes

  • A typo in the OHIP number or health number
  • Identifiers from two different patients mixed in one record

How to fix it

  1. Check the patient's current card and correct the identifiers on the claim.
  2. If the claim does not need an OHIP number, remove it and bill with the health number and version code only.
  3. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.

Don't resubmit if

  • You have not confirmed the patient's identifiers.

How APEX handles it
