Code
VS6
Description
Invalid HC Item Code Format

Explanation

Updated October 26, 2026

VS6 means the healthcare item code on a SEAMO claim is in the wrong format. Correct the code format and resubmit.

What it means

VS6 is one of a group of error codes (VS1 to VS6) for SEAMO claims. The Ministry lists it only as "Invalid HC Item Code Format".

SEAMO is the Southeastern Ontario Academic Medical Organization, one of the alternate funding arrangements named in the Schedule's Appendix Q. Its claims carry extra fields that ordinary fee-for-service claims don't.

How to fix it

  1. Check the healthcare item code format on the claim against the values set up for your SEAMO billing.
  2. Correct it and resubmit.
  3. If you are not sure which value is valid, ask the people who manage SEAMO billing for your department.

Don't resubmit if

  • The healthcare item code format has not been changed.

How APEX handles it

APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
