OHIP billing code for FIT test
Updated October 26, 2026
Family physicians bill Q150A ($7.00) for counselling a patient on colorectal cancer screening and sending the FIT requisition, and Q152A ($5.00) for reviewing the FIT result and telling the patient, each once every two years. FIT replaced the guaiac FOBT in Ontario's ColonCancerCheck program in 2019; the lab bills the test itself.
| Code | What it is | Fee | When to bill it |
|---|---|---|---|
| Q150A | Colorectal cancer screening fee: FIT counselling and requisition | $7.00 | You counsel an average-risk patient on screening and FIT use and submit the FIT requisition. Once per patient every two years. |
| Q152A | Colorectal cancer screening test completion fee | $5.00 | You review the FIT result and communicate it to the patient, if your payment model allows it. Once per patient every two years. |
| Q133A | Colorectal cancer screening tracking code | $0.00 | Billed at $0 to record that an enrolled patient has been screened, for preventive care bonus tracking. |
| Q142A | Colorectal cancer screening exclusion code | $0.00 | Billed at $0 to exclude an enrolled patient from the screening population, for example inflammatory bowel disease or a colonoscopy in the last 10 years. |
How to bill it
At the visit where you discuss screening, assess eligibility and history, confirm the patient's date of birth and mailing address for the kit, explain how to do the FIT, and submit the requisition. Bill Q150A with the visit (OMA Q-codes guide p.14).
When the result comes back, review it and tell the patient, then bill Q152A if your payment model allows it.
An abnormal FIT needs a follow-up colonoscopy. If your enrolment model offers it, Q043A is a new-patient fee for enrolling a patient referred to you for follow-up of an abnormal FIT (AQ1).
Billing rules
- From July 1, 2026, FIT screening starts at age 45 for people at average risk (formerly 50); the Ministry's target population is 45 to 74 (INFOBulletin 260702).
- Q150A and Q152A are each payable once per patient every two years. A second Q150A by another provider in that period pays $0 with explanatory code M4 (AQ4; OMA Q-codes guide p.14).
- After a negative colonoscopy, neither is payable again for 10 years unless earlier screening is clinically indicated (AQ4). ColonCancerCheck now returns people with 1–2 low-risk adenomas to FIT after 5 years (INFOBulletin 260702).
- Q150A can be billed by all primary care physicians, including those in patient enrolment models (AQ4).
- Q152A: Appendix Q says physicians in enrolment models who are eligible for the Preventive Care Bonus can't bill it, and FHG/CCM physicians can only if their roster is under the bonus minimum (650, or 450 for new graduates) (AQ4; OMA Q-codes guide p.14). The 2026 FHO agreement lists Q152A as payable to FHO physicians (Agreement p.72 §11.5).
- FHN and FHO physicians are no longer eligible for the colorectal cancer screening preventive care bonus, from fiscal 2024 bonuses billable April 1, 2025 onward (INFOBulletin 260401).
- Q150A and Q152A are payable with eligible comprehensive virtual care services (AQ11).
- Appendix Q is for information only and is not part of the Schedule (AQ1).
Common rejections
- M4: Maximum reached for these services (one or more practitioners). Q150A was already paid for this patient by any provider in the last two years, so the second claim pays $0 (AQ4; OMA Q-codes guide p.14).
Questions
What is the OHIP billing code for FIT test?
Family physicians bill Q150A ($7.00) for counselling a patient on colorectal cancer screening and sending the FIT requisition, and Q152A ($5.00) for reviewing the FIT result and telling the patient, each once every two years. FIT replaced the guaiac FOBT in Ontario's ColonCancerCheck program in 2019; the lab bills the test itself.
Is there still an OHIP code for FOBT?
ColonCancerCheck switched from the guaiac fecal occult blood test to FIT in June 2019 (INFOBulletin 4723). G004A ($1.58) still exists, but it is an occult blood test you do in your own office for your own patient with the result and treatment decision charted (J65, J67); it isn't the provincial screening test and isn't billed for sending a FIT.
Can I bill Q150A for a patient under 45 or with a family history?
Q150A is for average-risk screening. People at increased risk because of family history are screened by colonoscopy, starting at 40 or 10 years before the youngest relative's diagnosis, every 5 years (INFOBulletin 260702).
