Explanation
Updated October 26, 2026
Fee for counselling a patient on colorectal cancer screening and sending the FIT requisition. Once per patient every two years.
What it covers
Q150A pays for counselling a patient on colorectal cancer screening and the use of the fecal immunochemical test (FIT), and for completing and submitting the FIT requisition to the lab (OMA Q-codes guide p.13; Agreement p.72 §11.6). All primary care physicians in Ontario can bill it, including physicians in patient enrolment models (AQ4).
Appendix Q is for information only and is not part of the Schedule (AQ1). The rules below come from Appendix Q, the 2020 OMA Q-codes guide and the April 2026 FHO agreement.
Billing rules
- To claim it: assess the patient's medical and family history and eligibility for FIT, confirm date of birth and mailing address for the kit, teach correct FIT use during an office visit, and submit the requisition to LifeLabs by fax or certified EMR (OMA Q-codes guide p.14).
- For average-risk patients aged 45 to 74, enrolled or not (INFOBulletin 260702; OMA Q-codes guide p.14).
- Once per patient every two years (730 days) across all providers; a second claim by another provider in that period pays zero with explanatory code M4 (AQ4; OMA Q-codes guide p.14).
- After a negative colonoscopy (no polyps, or only hyperplastic polyps in the sigmoid or rectum), not again for 10 years unless earlier screening is clinically indicated by the specialist's findings or advice (AQ4).
- Minimum requirements are in INFOBulletin 4723 (AQ4).
- Payable with eligible comprehensive virtual care services (AQ11).
Often billed with
- The office visit at which you counsel the patient and teach FIT use (OMA Q-codes guide p.14).
- Q152A later, once the FIT result is reviewed and communicated, for physicians eligible to bill it (AQ4).
Common rejections
- EH2: The health card version code on the claim doesn't match OHIP's record for the patient; get the current version code and resubmit.
- AT3: Q150A can go on a comprehensive virtual care claim (AQ11), and AT3 means OHIP found no existing patient-physician relationship for that virtual claim on the service date.
- V23: OHIP flagged the number of services; this usually means a B- or C-suffix code was billed with one service, while Q150 is billed with the A suffix.
- V70: The Ministry's description for V70 is a service date later than the claim file or batch creation date; check the service date.
- VH1: The health number on the claim is blank or isn't a valid 10-digit Ontario health number.
