How the bonus is worked out

Coverage. Covered patients ÷ (patients on the report − excluded patients) × 100, rounded to two significant digits, as the Ministry's procedures document sets out.

One code per category. Claim the code for the highest tier you reached, billed at $0.00. The bonuses are not added together within a category.

FHO and FHN. Only the influenza (Q100A–Q104A) and childhood immunization (Q115A–Q117A) bonuses can be claimed since 2024–25. With fewer than 1,000 enrolled patients on March 31, each bonus is prorated: enrolled patients ÷ 1,000 × the bonus.

FHG and CCM. All five categories, with at least 650 enrolled patients on March 31 (450 for a new graduate in their first year in an FHG or CCM). Below that, no bonus is paid.

Other enrolment models. RNPGA, BSM, GHC, SJHC and WAHA physicians can claim all five categories with no minimum roster.

Questions

Can FHO physicians still claim the preventive care bonus?

Yes, but only for influenza (Q100A–Q104A) and childhood immunizations (Q115A–Q117A), since fiscal 2024–25. Cervical, mammography and colorectal claims from FHO and FHN physicians reject with EPA.

How is the bonus prorated for a small roster?

FHO and FHN physicians with fewer than 1,000 enrolled patients on March 31 get enrolled patients ÷ 1,000 × the bonus. For example, 575 patients at the 75% influenza tier (Q103A, $1,100) is $632.50.

What is the minimum roster for FHG and CCM?

650 enrolled patients on March 31 of the bonus year, or 450 for a new graduate in their first year in an FHG or CCM.

How do I bill the preventive care bonus?

Bill the tier code for each category at $0.00, one claim per category for the highest tier you reached, after the March 31 report and before the June claims cut-off. OHIP pays the bonus amount.
