Explanation

Updated October 26, 2026

Colorectal screening preventive care bonus, 40% tier: billed at zero when 40% of enrolled patients aged 45 to 74 are screened. Not listed for FHO or FHN.

What it covers

Q120A is the 40% tier of the colorectal cancer screening category of the cumulative preventive care bonus, which runs from Q118A (15%) to Q123A (70%) (AQ10-AQ11).

The target population is enrolled patients aged 45 to 74 inclusive on March 31 of the fiscal year being claimed (INFOBulletin 260805). The OMA guide measures compliance as a fecal occult blood test in the 30 months before that March 31 (OMA Q-codes guide p.17).

Appendix Q is information only and not part of the Schedule (AQ1).

Billing rules

  • Bill at zero (AQ10).
  • Appendix Q lists it for RNPGA, BSM, GHC, SJHC, SEAMO, WHA, and CCM and FHG physicians who meet the minimum roster size (AQ10).
  • FHO and FHN aren't listed for colorectal screening in Appendix Q (AQ10), and section 8.1 of the FHO agreement lets an FHO physician claim under only two preventive care headings, influenza vaccine and childhood immunizations (Agreement p.65 §8.1). The 2020 OMA guide still lists FHN and FHO; it is older than both, so Appendix Q and the agreement take precedence (OMA Q-codes guide p.17).
  • FHG and CCM physicians need at least 650 enrolled patients on March 31 of the bonus year, or 450 for new graduates in their first year in the model (OMA Q-codes guide p.15).
  • Physicians eligible for the preventive care bonus can't bill Q152A, the colorectal screening test completion fee (AQ4; OMA Q-codes guide p.6).
  • Screened patients are recorded with the zero-fee tracking code Q133A. Patients with known cancer followed by a physician, inflammatory bowel disease, a colonoscopy in the last 10 years, a history of malignant bowel disease, or a disease needing surveillance colonoscopies are excluded with Q142A (OMA Q-codes guide p.7, p.18).

Common rejections

  • V98: OHIP's description is 'wrong preventive care service date'. Appendix Q doesn't state the service date rule for the bonus claims; it refers to the Ministry's Primary Health Care fact sheets for complete billing information (AQ1). Correct the service date per the fact sheet and resubmit.
