General surgeon's consultation for a hospital in-patient, at the written request of a physician or nurse practitioner.

What it covers

C035 is a consultation for a non-emergency hospital in-patient in the General Surgery (03) listings (A107). The office version is A035.

A consultation is an assessment done at the written request of a referring physician, nurse practitioner or dental surgeon, because of the complexity, seriousness or obscurity of the case or at the patient's request for another opinion (GP16).

Billing rules

  • Needs a written, signed request from a physician, nurse practitioner or dental surgeon (in-hospital dental) naming you or your specialty, the referrer and their billing number, and the patient and health number; in hospital it may be in the common record. Otherwise it is paid as a lesser assessment (GP16).
  • Send a written report with findings, opinions and recommendations to the referrer (GP16).
  • Same patient, same diagnosis: one consultation per two consecutive 12-month periods (a second is allowed only for an in-patient or ED consult 12–24 months after the first); a clearly unrelated diagnosis allows one every 12 months. Extra ones are paid as an assessment (GP17, GP18).
  • Specialist consultations take the age premium for patients under 16 (GP64).
  • A consultation that becomes the admission assessment counts as such; only one consultation or assessment is payable per admission when care is transferred (GP40).
  • A pre-operative consultation before low-risk elective colonoscopy (or cataract, cystoscopy, carpal tunnel or arthroscopic surgery) under local or IV sedation is payable only if the record shows it was medically necessary (GP18).
  • For emergency calls and other special visits to in-patients, use the General Listings and special visit premiums (A107).

Often billed with

  • E082 MRP admission premium when the consultation is your admission assessment as most responsible physician (GP42).

Common rejections

  • ARF: OHIP describes this as 'Missing physician referring number': a consultation needs a written request from the referring physician or nurse practitioner, and their number belongs on the claim (GP16).
