Explanation

Updated October 26, 2026

Add-on for each additional polyp measuring 20 to 29 mm excised through the colonoscope, after the first-polyp code.

What it covers

E518 is an add-on for each additional polyp of 20 mm to 29 mm, listed under Z516 (excision of first polyp 20–29 mm) (S19).

The first polyp is billed with the code for the largest polyp removed (S19).

Billing rules

  • A maximum of 2 of E720, E520, E519 and E518, in any combination, per patient per day (S19).
  • Only one of Z571, Z518, Z517 or Z516 is payable per patient per day; use the largest polyp removed to choose the first-polyp code (S19).
  • Marked #: premises, equipment, supplies and staff are funded by the hospital budget or ICHSC facility costs, and patients can't be charged for them (GP13).
  • Under-16 age premiums (10–30%) apply to surgical procedures in Parts K to Z (GP64).

Often billed with

  • Z516 for the first (largest) polyp (S19).

Common rejections

  • ADF: OHIP describes this as 'Corresponding Procedure Invalid, Omitted or Paid at zero': the base procedure (Z516) is missing from the claim, invalid, or paid at zero. Add-ons are payable only with their base code.
  • A3H: Usually means more additional-polyp add-ons than allowed: at most 2 of E720, E520, E519 and E518 combined per day (S19).
