Weekly palliative care case management by the most responsible physician. One per patient per week; no G511, K071 or K072 that week.
What it covers
G512 pays for supervising a patient's palliative care for one week, starting at midnight Sunday. It covers monitoring the patient (including ordering and interpreting tests), phone advice to the patient, family or representative even when they call, and arranging assessments, procedures or therapy and coordinating community and hospital care, such as urgent palliative radiation or chemotherapy, blood transfusions, paracentesis or thoracentesis, and intravenous or subcutaneous therapy (J102).
The patient can be in any location, including home, hospital or a nursing home (J102). Palliative care is care in the final year of life with no aggressive treatment of the underlying disease, directed at comfort until death (GP5).
