Explanation

Chronic home care supervision: advice to Ontario Health atHome staff after the 8th week of home care. Up to 2 per physician per month.

What it covers

Home care supervision is personally giving medical advice, direction or information to staff of Ontario Health atHome or its contractor about a patient you give ongoing medical care to (A59).

K072 is the chronic rate, after the 8th week following the patient's admission to the Ontario Health atHome home care program; the first 8 weeks are K071 (A59).

Billing rules

  • Maximum 2 per patient per physician per month after the 8th week of home care (A59).
  • Maximum 4 per patient per month, from all physicians (A59).
  • Record the date and the advice, direction or information in the patient's chart, and the staff person's name if given verbally; without it the payment is nil (A59).
  • Pays nil on the same day as a consultation or visit by you with the same patient (A59).
  • Not payable to any physician in a week when G512 palliative case management is billed (J102).
  • G511 isn't payable for the same telephone call claimed as K072 (J101).
  • FHO: on the long-term care base-rate list (Agreement Schedule 2A, p.86) but not the community in-basket list (Schedule 2, p.79).

Common rejections

  • EH2: The health card version code doesn't match OHIP's record, often because the patient has a newer card; get the current version code and resubmit (EH2 guide).
