Pre-scheduled case conference about an out-patient under 18, led by a paediatrician or psychiatrist as MRP. Billed per 10-minute unit.
What it covers
A case conference is a pre-scheduled meeting, in person, by video or by phone, to discuss and direct the management of one patient. Teaching rounds and journal clubs are not case conferences (A31).
K704 is the paediatric out-patient case conference: the physician most responsible for the patient plus at least 2 other participants, who may be physicians, regulated social workers, regulated health professionals, education professionals or staff of a Children's Mental Health Ontario accredited centre, about an out-patient under 18 (A34).
Billing rules
- Only payable when the MRP is a paediatrician (26) or psychiatrist (19); other participating physicians of any specialty may bill it (A34).
- Units are 10 minutes of consecutive time: 1 unit = 10 min, 2 = 16, 3 = 26, 4 = 36, up to 8 units = 76 min (A31).
- At least 10 minutes of patient-related discussion; you must actively take part and your participation must show in the record (A32).
- Maximum 4 services per patient per physician per 12 months, and 8 units per patient per day (A34).
- No other case conference or telephone consultation for the same patient that day (A34).
- Not payable if the required participants are paid by you, or if you are paid other than fee-for-service (salary, APP, AFP) for the conference (A32).
- The record must show the patient, start and stop times of the discussion, the eligible participants and the outcome (A32).
Common rejections
- AM1: OHIP describes AM1 as 'Service Limit Exceeded': more than 4 K704 services for this patient in 12 months, or more than 8 units in a day (A34).
