Error code
AMR — Minimum service requirements have not been met
Minimum requirements not met
The service as billed doesn't meet a minimum the code sets, such as a minimum time or number of services.
What to do: Compare the code's requirements with the chart; if they weren't met, bill the code that matches what was done.
- Code
- AMR
- Description
- Minimum service requirements have not been met
Explanation
Updated October 26, 2026
AMR means the claim didn't meet the code's minimum requirements, such as a minimum time or number of services. Check the rules and bill to match.
What it means
AMR means the service as billed doesn't meet a minimum the Schedule sets for that code. The Ministry doesn't say which minimum; it depends on the code.
Common minimums are a minimum number of services (if fewer are done, none are payable, except in Diagnostic Radiology or where stated), a minimum time with the patient (start and stop times must be in the chart or the service isn't payable) (GP7), and a minimum number of visits for some management fees (for example W010 needs at least two W-prefix assessments in the month, GP51).
Common causes
- Time with the patient was below the code's minimum, or start and stop times weren't charted
- Fewer services than the code's minimum
- A management fee billed before its required visits were done
- The number of services or units on the claim is wrong
How to fix it
- Read the code's requirements in the Schedule.
- Compare them with your chart: times, number of services, visits in the period.
- If the requirements were met, correct the claim so it shows them (for example the right number of services or units).
- If they weren't met, bill the code that matches the service you actually provided.
- Resubmit the corrected claim within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- The requirements weren't met and no other code fits. Sending it again will reject again.
How APEX handles it
APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
