- Code
- D8
- Description
- Allowed with specific procedures only
Explanation
Updated October 26, 2026
OHIP didn't pay this code because it can only be paid with certain procedures, and none of them was billed with it.
What it means
Code D8 means the code is allowed only with specific procedures. Add-on codes and premiums are payable only with the base codes named in the Schedule.
For example, the BMI over 40 surgical premium (E676) is payable only with the procedure codes listed in the Surgical Preamble (SP6).
Common causes
- An add-on or premium billed with a procedure it can't be paid with
- The base procedure billed on a different claim or date
How to fix it
- Check the Schedule listing for the code to see which procedures it can be paid with.
- If one of those procedures was billed with it, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with supporting details such as your chart notes.
- Otherwise, the code isn't payable.
Don't resubmit if
- None of the allowed base procedures was done.
