EQ1 — Provider not registered with OHIP
Practitioner not registered

What to do: Check the billing number for typing errors; for a new physician, confirm their OHIP registration covers the service date.
- Code
- EQ1
- Description
- Provider not registered with OHIP
Explanation
Updated October 26, 2026
EQ1 means the billing practitioner isn't registered with OHIP. Check the billing number on the claim and the registration.
What it means
EQ1 means the clinic or doctor on the claim is not on file: the practitioner is not registered with OHIP. The claim was rejected.
Common causes
- A typing error in the billing number
- A new physician whose OHIP registration isn't complete
- The service date is before the registration took effect
How to fix it
- Check the billing number on the claim.
- If it was mistyped, correct it and resubmit.
- If the physician is new, confirm their OHIP registration is complete and in effect for the service date, then resubmit. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process.
Don't resubmit if
- The practitioner's registration isn't in effect yet.
