Description
Contract characteristics error

Explanation

Updated October 26, 2026

EQI is a contract characteristics error: the claim doesn't fit your registration or payment agreement on the service date. Check your registration.

What it means

EQI means the claim did not match the characteristics of the contract or registration the Ministry has on file for you (or your group) on the date of service. The Ministry gives no further detail for this code.

How to fix it

  1. Check the billing number, group number and specialty code on the claim.
  2. Check that the fee code is one your contract or payment model allows on that date.
  3. If everything looks right, contact the Ministry to find out which contract detail failed.
  4. Resubmit the corrected claim within three months of the service date. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process.

Don't resubmit if

  • You have not found what failed. The same claim will reject again.

How APEX handles it

APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
