EQE — Provider not registered in this group on service date

- Code
- EQE
- Description
- Provider not registered in this group on service date
Explanation
Updated October 26, 2026
EQE means the physician on the claim isn't registered with the Ministry as a member of the group whose number was used.
What it means
Claims billed under a group number must come from a physician registered as an affiliate of that group on the service date. EQE means the Ministry has no record linking this physician to the group for that date.
EQF is the related code for a physician who is linked to the group but whose membership wasn't active on the service date.
Common causes
- A new physician or locum billed under the group before their membership was registered.
- The claim used the wrong group number, for example a different clinic's group.
- The physician belongs to a different group than the one in the billing software.
- The group membership form was never submitted or wasn't processed.
How to fix it
- Check that the group number on the claim is the group the physician belongs to.
- If the number was wrong, resubmit under the correct group number, or the solo billing number.
- If it's the right group, confirm with the group lead and the Service Support Contact Centre (1-800-262-6524) that the membership is registered. Since April 1, 2026, the consent and authorization for group membership is submitted through the Ministry's online registration application.
- Resubmit after the membership is in place for the service date. Older claims may need to go as stale-dated.
Don't resubmit if
- The physician still isn't registered with the group and you're using the same group number.
- The physician was never a member of that group. Use the number they're actually registered under.
How APEX handles it
For family practice claims, APEX resubmits EQE claims as billed, with the patient's date of birth taken from a health card check. This only works once the physician's group membership is registered for the service date, so confirm that before approving. See how APEX works · Pricing
