Explanation
Updated October 26, 2026
EV on your remittance advice is a reminder that the patient's health card version code may be out of date. Update it before the next claim.
What it means
EV is an explanatory code on the Remittance Advice, not an Error Report rejection. Remittance Advice codes explain payment exceptions on claims OHIP has processed. EV tells you to check the patient's health card for the current version code.
In practice, the version code on file for this patient is likely outdated, often because a replacement card was issued. If you keep billing with the old version code, later claims are likely to reject with EH2. A related code, EF, warns that services on or after the 20th of the month won't be paid unless the current version code is provided.
Common causes
- Patient got a replacement card with a new version code
- Chart or billing software still holds an old version code
- Version code entered wrong at registration
How to fix it
- Look at the amount paid on the line. EV on its own doesn't mean the claim was rejected.
- At the patient's next visit, or now, check their card or run a health card validation check to get the current version code.
- Update the version code in the patient's chart so future claims carry the right one.
- If the line was not paid and you believe it should have been, contact OHIP about the claim rather than sending it again as new.
Don't resubmit if
- The line was paid as expected. Update the version code for future claims; there's nothing to resubmit.
- You'd be sending the same claim again. A claim already on the Remittance Advice isn't corrected by resubmitting it.
How APEX handles it
APEX treats EV as a version-code (eligibility) issue, not an automatic fix. It parks these lines on the same eligibility (EH2) table as EH2 claims, so the patient's current version code can be looked up. See how APEX works · Pricing
