- Description
- Incorrect version code. Services on or after the 20th of this month won't be paid without the current version code
Explanation
Updated October 26, 2026
EF warns the version code on the claim is wrong. Services on or after the 20th of this month won't be paid without the current version code.
What it means
EF appears on the Remittance Advice (RA) when the health card version code on the claim is incorrect. It also warns that services on or after the 20th of this month will not be paid unless the current version code is provided.
The two-letter version code changes when a new card is issued. The patient is often still covered and just has a newer card. This RA code is not related to the EF1 to EF9 error codes, which are for community health services centres.
Common causes
- The patient has a newer card and the chart still has the old version code
- A typing error in the version code
How to fix it
- Check the amount paid on the line.
- Ask the patient for their current card at the next visit, or run a health card validation check, and update the chart.
- If you can't get the card from the patient, physicians registered as health information custodians can request the health number and version code from ServiceOntario's Health Number Release service (the 24x7 Help Desk or form 1265-84).
- Use the current version code on all future claims for the patient, especially services on or after the 20th of the month.
- If the patient never received a new card, ask them to contact ServiceOntario.
Don't resubmit if
- The claim is already on your RA. A new claim for the same service will be treated as a duplicate. Use an RAI instead.
