- Code
- VH4
- Description
- Invalid Version Code
Explanation
Updated October 26, 2026
VH4 means the version code on the claim isn't valid. Get the version code from the patient's current card and resubmit.
What it means
VH4 means OHIP did not accept the version code sent with the health number. The version code is the one or two letters after the 10-digit health number on the health card. It changes when a new card is issued.
The Ministry's list gives only the short description "Invalid Version Code". When the version code is well-formed but does not match the Ministry's record, the claim usually rejects as EH2 instead.
Common causes
- A typo in the version code
- The patient has a newer card and the chart has the old version code
- Characters other than letters in the version code field
How to fix it
- Ask the patient for their current card, or run a health card validation check.
- Update the version code in the chart and on the claim.
- Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- You haven't found a new version code. Sending the same one again will reject again.
How APEX handles it
APEX reads your error reports and remittance advice from MCEDT and lists claims rejected with this code in its Correction Centre for you to correct and resubmit. See how APEX works · Pricing
