- Code
- H8
- Description
- Hospital number and/or admission date required for in-hospital service
Explanation
Updated October 26, 2026
H8: an in-hospital service needs the hospital (master) number and/or the admission date on the claim. Add the missing details.
What it means
H8 means the claim was for an in-hospital service but did not include the hospital number (master number) and/or the in-patient admission date that OHIP needs to pay it.
The Ministry's claim specifications require the master number and the in-patient admission date for non-emergency hospital in-patient services, except consultations, repeat consultations and limited consultations. The admission date must be the same as or before the service date.
Common causes
- The master number was left off an in-patient claim
- The admission date was left off, or is after the service date
How to fix it
- Get the hospital's master number and the patient's admission date.
- Add them to the patient record in your billing software so future claims include them.
- For this claim, follow your RA. If it was not paid, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date, with the master number and admission date.
