Explanation
Updated October 26, 2026
HF: concurrent or supportive care was already claimed for this patient in the period, so this visit wasn't paid. The weekly limits apply.
What it means
HF means concurrent or supportive care had already been claimed for this patient in the same period, so this claim was not paid.
Concurrent care is a routine in-hospital assessment by a consultant after their first major assessment, when the family physician stays the most responsible physician and asks the consultant to keep directing care. Supportive care is a routine in-hospital visit by a family physician who is not treating the case, at the patient's or family's request, for liaison or reassurance. Each is limited to 4 claims in the first week and 2 per week after that (Schedule GP48).
Common causes
- More than 4 concurrent or supportive care visits in the first week, or more than 2 a week after that
- Concurrent or supportive care already claimed for the same period by you or another physician
How to fix it
- Count the concurrent or supportive care visits billed for the week.
- If the claims are within the limits, send a Remittance Advice Inquiry (RAI) through eSubmit, or on form 0918-84, within 7 months of the RA date.
Don't resubmit if
- The claim is already on your RA. Use an RAI, not a new claim, if you disagree.
