Error code
V68 — Incorrect service location indicator
Admission date without HIP
The diagnostic claim had a hospital master number and an admission date, but its service location code wasn't HIP, so they contradict.
What to do: If the patient was admitted on the service date, set the location code to HIP; if not, remove the admission date.

Explanation
Updated October 26, 2026
V68 means a diagnostic claim had a master number and admission date but its SLI was not HIP. Fix the SLI, or remove the admission date, and resubmit.
What it means
V68 means a diagnostic service was billed with both a hospital master number and an admission date, but the service location indicator (SLI) was not HIP. The Ministry treats an admission date as a sign the patient was admitted, so the SLI and the admission date contradict each other.
The service location indicator (SLI) is a three-letter code on the claim that tells OHIP where the service was provided. For hospital diagnostic service billings, the Ministry recognizes five hospital SLI codes: HDS, HED, HIP, HOP and HRP. Each of these also needs the hospital's master number on the claim.
Common causes
- The patient was an in-patient but the SLI was left at an out-patient, emergency or other code
- An admission date was carried into a claim for a patient who was not admitted on the service date
How to fix it
- Check whether the patient was admitted on the service date.
- If yes, set the SLI to HIP and keep the master number and admission date.
- If no, remove the admission date and keep the SLI that matches where the service was done.
- Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- Nothing on the claim has changed. The same SLI and admission date will reject again.
