- Code
- VH3
- Description
- Invalid Payment Program
Explanation
Updated October 26, 2026
VH3 means the claim's payment program was missing or not HCP, RMB or WCB. Set the right payment program and resubmit.
What it means
VH3 means the payment program field on the claim was blank or held something other than the three values OHIP accepts: HCP, RMB or WCB.
HCP is for patients with Ontario health coverage. RMB (Reciprocal Medical Billing) is for patients from other provinces, except Quebec. WCB is for Ontario-covered patients with work-related injuries.
Common causes
- The payment program was left blank
- A billing software setting or import produced an unsupported value
How to fix it
- Pick the payment program that fits the patient: HCP, RMB or WCB.
- Check the fields that go with it: an Ontario health number for HCP and WCB, or Claim Header 2 with the provincial registration number for RMB.
- Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.
Don't resubmit if
- The payment program is still blank or not one of HCP, RMB or WCB.
