VH2 — Health Number is Missing

Missing health number

The claim was billed under HCP or WCB, which both need an Ontario health number, but the health number was blank.

What to do: Add the patient's health number and version code from their card or a validation check, then resubmit.

Code
VH2
Description
Health Number is Missing

Explanation

Updated October 26, 2026

VH2 means the claim had no health number but the payment program was HCP or WCB, which need one. Add the patient's health number and resubmit.

What it means

VH2 means the health number field was blank on a claim billed under the HCP (Ontario health insurance) or WCB (workplace injury) payment program. Both programs are for patients with Ontario coverage, so the claim needs the patient's Ontario health number.

Common causes

  • The health number was never entered for the patient
  • An out-of-province patient was billed under HCP instead of RMB
  • The health number was lost when the patient record was edited or merged

How to fix it

  1. Get the patient's health number (and version code) from their card or a health card validation check.
  2. Add it to the claim.
  3. If the patient is from another province (not Quebec), bill under RMB with their provincial registration number instead.
  4. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.

Don't resubmit if

  • You still don't have a health number for the patient.
