VHO — Health number and Claim Header 2 on same MRI claim

Health number with Header 2

Same as VH0: the claim had an Ontario health number and also a Claim Header 2 record meant for out-of-province patients.

What to do: If the patient had Ontario coverage, bill under HCP with the health number and remove the Claim Header 2 details.

Code
VHO
Description
Health number and Claim Header 2 on same MRI claim

Explanation

Updated October 26, 2026

VHO is the same rejection as VH0: an Ontario health number and a Claim Header 2 on one claim. Keep only the one that fits and resubmit.

What it means

VHO is read the same way as VH0 (with a zero), the code the Ministry lists as "Header 2 and HN Present". It means the claim carried an Ontario health number in Claim Header 1 and also had a Claim Header 2 record. Claim Header 2 is used for Reciprocal Medical Billing (RMB) claims for out-of-province patients.

A claim for an Ontario patient with a health number should not have Claim Header 2.

Common causes

  • An Ontario patient was billed with out-of-province details still attached
  • The patient moved to Ontario (or away) and old registration details were left in the chart

How to fix it

  1. Decide whether the patient had Ontario coverage on the service date.
  2. If yes, bill under the HCP payment program with the health number and remove the Claim Header 2 details.
  3. If the patient is from another province (not Quebec), bill under RMB with their provincial registration number in Claim Header 2 and no Ontario health number.
  4. Resubmit the corrected claim. Resubmit within three months of the service date if you can. If that window has passed but the claim was first submitted on time, use the Ministry's stale-dated resubmission process: one stale-dated claim file through MCEDT, then an email to [email protected] with the Error Reports showing the original submission date.

Don't resubmit if

  • The claim still has both a health number and Claim Header 2.
